Azara Healthcare provides data reporting and education to support quality improvement initiatives for participating practices in New York state
Azara Healthcare, the top-rated provider of population health solutions and the American Lung Association, the leading organization working to save lives by improving lung health and preventing lung disease, have partnered to improve the health of patients suffering from asthma in New York state.
Azara and the Lung Association have partnered to provide data reporting and analytics to support the Lung Association’s state-wide efforts to promote and communicate asthma-related best practices and their Asthma Control Program, Project BREATHE NY. Project BREATHE NY is an innovative quality improvement program which aims to improve quality of life and health outcomes for children with asthma and their families by expanding the delivery of quality, guidelines-based asthma care across health systems in New York state.
The partnership also aims to expand the availability and use of an asthma-related registry and measure reporting available through Azara DRVS and disseminating Azara-related resources to participating and newly recruited sites. Participating providers are able to use asthma data to identify quality improvement opportunities and develop strategies to communicate gaps in care with other nearby providers (community-based organizations, specialty providers, hospitals, home care agencies, etc.). Together, Azara and the Lung Association developed a strategy to integrate data from existing practices successfully using Azara DRVS with local health systems sites—sharing healthcare utilization data to achieve better coordination of care, care gap closure, and overall patient health.
“Through the partnership with the Lung Association and Azara, we have seen participating practices better able to map and validate asthma measures they are currently collecting in their Electronic Health Record (EHR) to monitor progress, educate staff on additional opportunities for asthma education, and ultimately, provide higher patient care,” said Alexandra Kranidis, Director of Health Promotion at the American Lung Association. “We look forward to continuing this work and expanding partnerships to additional practices in New York.”
Together, Azara and the Lung Association are committed to providing continued education, quality improvement, and professional development in the communities served. The Lung Association will also provide scholarships for the American Lung Association’s Asthma Educator Institute and Asthma Educator Certification for participating providers.
